Symphony Healthcare Services (SHS) have an exciting opportunity that has arisen to join a forward-thinking practice based at Martock and South Petherton Surgeries in Somerset. Funding is available to up-skill the right candidate through accredited courses to enhance their skill set.

As a Symphony Practice Nurse you will:

be part of a nursing team responsible for ensuring that the practice provides optimum care relevant to the practice population, either in practice or the home environment.

focus on supporting patients to be healthy, monitoring long-term conditions, health prevention and screening activities in line with the objectives and vision of Symphony Healthcare Services.

work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

This is a part time post of 30 hours per week, on a permanent contract.

The annual salary range for this post is £39,146 – £41,851per annum (pro rata for part time hours).
