Key 2026/27 QOF Changes & Additions

- Obesity (New Indicators): Replaces the retired Weight Management ES.
  - - OB004 (5 points): Requires referring patients (aged 18+ with a BMI \(\ge 30\) or \(\ge 27.5\) for BAME groups) to a structured weight management program within 90 days of the BMI recording.
    - OB005 (13 points): Focuses on shared decision-making and pharmacotherapy for obesity. [1, 2, 3]

Childhood Vaccinations (VI001, VI002, VI003): Practices can now earn points through either traditional achievement thresholds or a sliding scale based on improvement from a 2-year baseline. [1]
Diabetes (DM037): A new indicator for the delivery of all 8 NICE-recommended diabetes care processes. [1, 2]
Heart Failure (HF009): Indicators have been updated to reflect the NICE-recommended “4 pillars” of treatment. [1, 2]
Asthma: Clinical indicators have been expanded to include children aged 5+ (previously 6+). [1, 2]
Atrial Fibrillation (AF006): The threshold for anticoagulation in AF patients has been increased from \(90\%\) to \(95\%\). [1]
