Somerset Interface Issues Reporting Form
A new Somerset Interface Issues Reporting Form has been launched to capture concerns around the interface between General Practice and Somerset NHS Foundation Trust (SFT). This form has been designed to be quick and easy to complete. The aim is to achieve a high report rate and identify recurrent problems. Issues like onward referrals, urgent prescription requests, inaccurate discharge summaries can all be reported here. This does not replace the need for your practice significant event process for any issues that reach that threshold (although they can also be reported on the form if you wish).
The Somerset Interface Group won’t respond to individual submissions but feedback will be provided on the themes identified and actions taken to try to improve them. Please avoid including any patient or staff identifiers.
Please encourage your practice teams to use this form to report ALL interface issues that you come across. This will give us a better picture of where the biggest issues are so that we can focus our energy on tackling the most frequent issues first.
